Leading Edge Safety Systems was engaged by Networks NSW to identify Network Fatal Risk Controls across Ausgrid, Endeavour Energy and Essential Energy. Fatal risks are high consequence, low frequency life changing events that may result in loss of life and perma nent disabilities. Nine fatal risks were identified through technical working groups comprising a cross section of functional expe rti se from the three NSW network businesses. The bow tie risk management methodology was used to evaluate each of the fatal risk events, demonstrate key causes, the potential consequences of major accidents, and to identify both prevention and mitigation risk co ntr ols to further improve the efficacy of the risk management approach adopted across the three Network businesses.
From this work a Network Fatal Risk Control Standards document was produced, which outlines minimum controls to be in place to prevent fatalities or disabling injuries on the Network. These fatal risk controls now underpin the key safety assurance acti vit ies performed across each of the three Network businesses, including incident investigation, auditing and upwards safety reporting to the E xec utive.
