Ergon Energy (EE) and Energex (EX) were brought into the Energy Queensland group as at June 2016. As part of this change EQHSE Investigators/Auditors across the previous two groups were integrated and an initial focus was to ensure consistency in the application of the ICAM incident investigation process. A November 2017 review conducted by Dr Graham Edkins of 10 EE and EX ICAM reports, revealed some inconsistencies in the identification of individual/team actions and the determination of likely predisposing conditions that influenced the behavior of those directly involved parties being investigated. It was apparent that both organisations’ reports followed different analytical methodologies, which may reflect different understandings (and previous training provided) of human factors and error management principles. A focused Human Factors and Error Management workshop program was developed to align thinking in the concept of human factors, human error and error tolerance as it applies to the conduct of an EQHSE incident investigation.
A tailored program was successfully rolled out in Townsville and provided a shared understanding and alignment of thinking amongst participants about human factors, human error and error tolerance, as it applies to the conduct of a systemic incident investigation within Energy Queensland.
